黄熱予防接種（YellowFeverVaccination）
黄熱予防接種をご希望の方へ
当院では2026年4月より、黄熱病の予防接種を開始しました。
対象地域への渡航前に接種をご希望される方は、下記をご参照の上、ご予約ください。
黄熱ワクチンは接種10日後から有効となります。
予約状況により、ご希望の日程で予約取得できないことがありますので余裕を持って予約をお願いいたします。

• The Yellow Fever vaccine becomes effective 10 days after vaccination.

予約前の注意事項 （Important Information Before Reservation）
接種できない方 （Individuals Who Cannot Receive the Vaccine）
- 9ヶ月未満の乳児（Infants under 9 months of age）
- 4週間（27日）以内に他の生ワクチンの接種している方
（Those who have received another live vaccine within the past 4 weeks (27 days)） - 妊娠中あるいは妊娠している可能性のある方は、原則接種できません。また、接種後2ヶ月間は妊娠を避けるようにしてください。
（Pregnant individuals or those who may be pregnant.
Pregnancy should be avoided for 2 months after vaccination) - ステロイドや免疫抑制薬の内服など免疫不全状態にある方
（Individuals with immunodeficiency, including those taking steroids or immunosuppressive medications） - 胸腺に関連した疾患（重症筋無力症、胸腺腫）、及び胸腺摘除術を受けた方
（Individuals with thymus-related disorders
(e.g., myasthenia gravis, thymoma) or those who have undergone thymectomy）
接種にあたり注意が必要な方 （Individuals Requiring Special Caution）
- 持病のある方は、事前に医師に予防接種の可否をご相談ください。
（Those with chronic medical conditions should consult their physician in advance） - 60歳以上の方は重い副反応が出る可能性があります。医師とご相談の上、接種ください。
（Individuals aged 60 years or older may have a higher risk of serious adverse reactions
→ Vaccination should be discussed carefully with a physician） - 授乳中の方は、乳児への安全性が確立していないため、授乳を1ヶ月は控えてください。
（Breastfeeding individuals should refrain from breastfeeding for 1 month after vaccination） - 日本語を話せない方は通訳の方につきそっていただくか、翻訳が可能な手段をもって予約と来院をお願いします。
（If you do not speak Japanese, please be accompanied by an interpreter or ensure that you have access to a means of translation when making a reservation and visiting the hospital.）

接種料金 （Vaccination Fee）
黄熱ワクチン接種料は、収入印紙にてお支払い頂きます。
料金：17,680円（黄熱ワクチン接種料・国際証明書料）
17,680 JPY(Includes Yellow Fever vaccination fee and International Certificate of Vaccination)
※ 現金やクレジットカード等はご利用になれません。
※ 収入印紙に対しておつりを出すことが出来ないため、規定額の収入印紙をご用意ください。
※ 家族等の接種の場合、合算額でなく一人ずつ規定額の収入印紙をご用意ください。
（接種不適当者がいた場合、返還困難なため)
※ 収入印紙は、事前に郵便局等でご購入ください。
当院最寄りの郵便局（庄郵便局）は約600m離れた場所にあります。
Payment Method
Payment must be made using revenue stamps (Japanese revenue stamps) only
Cash, credit cards, and electronic payments are not accepted
Please prepare the exact amount (no change can be given)
For multiple family members, revenue stamps are required per person
Revenue stamps must be purchased in advance at a post office
The nearest post office (Sho Post Office) is approximately 600 meters from the hospital.
当日持参するもの （Items to Bring on the Day of Vaccination）
接種日当日は下記をご持参ください。
- 収入印紙（１名分／17,680円）
- パスポートの顔写真のページのコピーもしくは原本（パスポートのない場合は、運転免許証等の身分証明書）
- 接種記録を希望される方は母子手帳（成人はお持ちの方）あるいは過去の予防接種記録
- お薬手帳（内服治療中の方）
- 診察券（当院受診歴のある方）
Please bring the following on your appointment day:
- Revenue stamp (17,680 JPY per person)
- Passport (original or copy of photo page)
- If unavailable, another form of government-issued ID
- Maternal and Child Health Handbook or previous vaccination records (if available)
- Medication notebook (if currently taking medication)
- Hospital ID card (if you have visited our hospital before)
受診の流れ
ワクチンに関する注意事項 （Visit Flow and Important Notes）
- 18歳未満の方は、保護者同伴で受診してください。
（Individuals under 18 years old must be accompanied by a parent or guardian） - 当日に発熱や風邪症状があると、接種ができない場合があります。
（Vaccination may not be administered if you have fever or cold symptoms on the day） - ご自宅を出発する前に体温測定を行い、キャンセル希望・体調不良等あれば速やかにご連絡ください。
（Please measure your body temperature before leaving home. If you feel unwell or wish to cancel, contact us immediately） - 予約時間に大幅に遅れると接種できないことがありますのでご注意ください。
（Arriving significantly late may result in cancellation） - 医師の問診の結果、接種できない場合がありますのでご了承ください。
（Vaccination may be declined based on the physician’s medical assessment） - 接種後30分間は、副反応の確認をするため待機していただきます。
待機していただいた後、黄熱予防接種国際証明書を交付します。
（After vaccination, you will be required to wait for 30 minutes for observation
After observation, the International Certificate of Vaccination will be issued） - 予防接種当日の飲酒及び激しい運動は控えてください。
（Avoid alcohol consumption and strenuous exercise on the day of vaccination）
予防接種禁忌理由書 （Notice of Vaccine Contraindication）
医師が診察し、接種希望者が接種不適当または要注意の場合（接種による副反応のリスクが感染リスクを上回ると考えられる場合）には、予防接種禁忌理由書の発行が可能です。
手数料は5,500円（税込）です。
ただし、予防接種禁忌理由書は国によってこれを受け付けず、入国できない場合もあります。
事前に大使館へ確認することをお勧めいたします。
If a physician determines that vaccination is contraindicated or requires caution
(when the risk of adverse reactions outweighs the infection risk), Notice of Vaccine Contraindication can be issued.
Fee: 5,500 JPY (tax included)
Please note that some countries may not accept Notice of Vaccine Contraindication and may deny entry.
We strongly recommend confirming requirements with the relevant embassy in advance.
国際証明書を紛失した場合 （Lost International Certificate of Vaccination）
